Registered Nurse: On-Call

JOB DESCRIPTION SUMMARY

The on-call nurse is a registered nurse who is available outside of regular business hours to answer patient calls, assess medical situations over the phone, provide advice and care instructions, and determine if a patient needs to be seen by a healthcare provider immediately. The on-call nurse acts as a first point of contact for urgent patient needs.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Telephone Triage

• Assesses patient symptoms and concerns over the phone to determine the urgency of the situation and appropriate course of action.

Communication

• Determines if a patient requires an immediate in-person visit and when necessary, completes the visit.

Care Coordination

• Communicates with other members of the home health and hospice teams as appropriate.

Patient Education

• Provides health information and instructions regarding home care management for various conditions

Documentation

• Maintains detailed records of patient calls, assessments, and interventions taken.

POSITION QUALIFICATIONS

• Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
• Current licensure in State and CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Bachelor’s degree, with one (1) year of hospice care experience preferred.
• Management experience not required. Responsible for supervising hospice aides.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
