Key Responsibilities as an Occupational Therapist:
- Perform Occupational Therapy Assessments: Conduct comprehensive OT assessments and skilled procedures aligned with the patient’s Plan of Care.
- Complete OASIS Assessments: Ensure OASIS assessments are completed within 48 hours of referral or as directed.
- Develop Therapy Plans: Create personalized Therapy Plans of Care and collaborate with healthcare professionals to ensure optimal patient care.
- Attend Case Conferences: Participate in case conferences, team meetings, and required educational programs to stay updated on occupational therapy best practices.
- Communicate with Care Teams: Notify the Primary Care Physician (PCP), Clinical Manager, and other team members of any changes in the patient’s condition for seamless care.
- Educate and Support Patients: Provide tailored education to patients based on their environment, needs, and learning ability to promote recovery and symptom management.
- Utilize OASIS Tools: Leverage OASIS tools to enhance patient outcomes and ensure high-quality care.
- Follow Infection Prevention Protocols: Adhere to Infection Prevention and Control policies to create a safe environment for both patients and staff.
- Supervise Home Health Aides: Oversee the work of Home Health Aides in compliance with Agency policies to ensure care quality.
- Ensure Compliance: Maintain adherence to state, federal, and agency regulations within the Occupational Therapy Scope of Practice.
- Pursue Professional Growth: Take responsibility for your personal and professional development by identifying learning opportunities with supervisors.
Required Qualifications for an Occupational Therapist:
- Occupational Therapist License (OT): Must be licensed as an OT in South Dakota.
- Healthcare Experience: Minimum of 2 years of experience in healthcare, preferably in home health care.
- CPR Certification (Preferred): Current CPR certification is preferred but not mandatory.
- Strong Communication Skills: Excellent verbal and written communication skills with the ability to work independently.
- Valid Driver’s License: Must hold a valid driver’s license with auto insurance and a clean driving record.
- Background Check: Must be able to pass a background check.
